Application for endorsement

Dear Chaplain candidate:

Please read the copy of the Requirements and Procedures for Endorsement for all Chaplaincies and Membership in Professional Societies. You will then need to submit an application form, and a supplemental form for institutional chaplaincy and pastoral counselors.

When filling out the application form, under the heading References, be sure to list those persons who know you well and are apt to answer correspondence, and be sure to list as one of those persons your regional minister. You also will need four Confidential reference forms to give to the persons you list as references to be completed and returned directly to this office.

A recent photo of you and/or your family would be helpful for me to get to know you, however the photo is optional. A brief biographical sketch, copy of your ordination certificate and undergraduate and graduate transcripts are required.

Once we receive all of the paperwork, including your four references, your file can be presented to the Committee on Chaplaincy Endorsement and Services for their consideration.
